Your Questions, Answered: Everything You Need to Know About Chronic Care Management

Pain Management Remote Monitoring is a distinct service that complements your existing telehealth offerings. It allows for monthly recurring reimbursements through specific CPT codes and is not tied to individual visits.

Based on your area of care, our team helps identify clinically eligible patients. General eligibility guidelines include:

  • Patients may have a chronic pain condition (e.g., arthritis, neuropathy) or acute pain episodes. Most participants typically have at least one long-term pain-related condition.

  • Participants must be established patients who have previously received an evaluation and management service, either in person or via telehealth.

  • Patients currently enrolled in another pain management remote monitoring program are not eligible.

  • Nursing home residents are eligible to participate in the program.

Most patients’ insurance plans fully cover Pain Management Remote Monitoring benefits with no co-pays. Depending on the insurance provider, some patients may have a small financial responsibility. Our team obtains patient consent regarding any potential costs before enrollment in the program.

Patients can stay enrolled in the Pain Management Remote Monitoring program as long as they continue to meet clinical eligibility criteria and their insurance coverage remains active. Participation is ongoing to ensure continuous monitoring and optimal pain management support.
