• Most patients enrolled in RTM have a musculoskeletal condition, chronic pain, or need post-surgery MSK monitoring.
• RTM program members should have a prior management and/or evaluation relationship with their provider in person or via telehealth.
• Patients may not be currently enrolled in another RTM program.

• Many patients have $0 out-of-pocket expenses.
• Depending on insurance plans, some patients may have minimum financial responsibility, such as co-pays.
• MoonMd will contact the patient with an estimate of their copay for approval before final enrollment in the program.

Patients can only be enrolled in one RTM program by a provider at any given time. During enrollment, **MoonMD** confirms with patients that they are not already participating in another RTM program.

Patients enrolled in **RTM** can also join a **Chronic Care Management (CCM)** or **Principal Care Management (PCM)** program through **MoonMD**, which supports better outcomes for comorbid conditions and is billed separately under distinct CPT codes.

Patient engagement is managed either by your own staff or by MoonMD’s remote monitoring support, ensuring adherence and timely check-ins. Staff are notified only when necessary, based on risk stratification alerts, and notifications occur strictly during the provider’s office hours.
